Hospital: Greenwich Hospitalist

Location: 5 Perryridge Road, Greenwich, CT

Scheduled Hours: 13 shifts per 4 week period (3/3/4 schedule, 12-hour shifts)

Yale New Haven Health is seeking an Acute Care-trained Advanced Practice Provider to join the hospitalist team at Greenwich Hospital in Greenwich, Connecticut. In this role, you will play an integral part in delivering high-quality, timely care to patients throughout the hospital, working alongside a collaborative team dedicated to excellence in patient care.

As a valued member of our team, you will:

  • Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
  • Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
  • Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.
  • Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.
  • Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma).
  • Perform minor office procedures as trained and within scope of practice.
  • Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care.
  • Document all patient encounters accurately and thoroughly in the electronic health record.
  • Participate in quality improvement initiatives and adhere to all organizational policies and procedures.
  • Serve as a resource for medical staff and participate in educational activities as needed.
  • Provide age-appropriate care to patients across the lifespan, from infants to geriatrics.
  • Manage referrals to specialists and coordinate follow-up care.
  • Maintain current knowledge of family medicine best practices and engage in ongoing professional development.

Qualifications

Qualifications:

  • 1-2 years clinical experience in a of experience in primary care/internal medicine setting preferred but not required.
  • Active PA or NP - Acute Care required
  • Active, unrestricted CT medical license or in process of obtaining
  • ACLS/BLS/PALS (as required) Certification
  • DEA License
