Full-Time; FTE 1.0

Monday-Friday; 8:00am-4:30pm

Rock Island, IL

The Registered Nurse (Clinic) is supervised by the Manager, Clinical Community Support Community Support Program and provides care to clients utilizing the Robert Young Center principles of Trauma Informed Care. The Registered Nurse (RN) provides case management services to severely mentally ill adults. These services include but are not limited to assessments, treatment planning, supportive counseling, medication monitoring, and linkage with various community resources. The RN functions as a Qualified Mental Health Professional (QMHP) according to the Illinois Register Department of Healthcare and Family Services 89 Ill. Adm. Code 140, and provides supervision to Mental Health Professionals (MHP) and may train students assigned in the department. The RN delivers customer service and works cooperatively with other departments. The RN provides crisis intervention, maintains accurate clinical documentation, and demonstrates Trinity's principles and values when interacting with clients, families, visitors, physicians and coworkers.


Responsibilities

Case Management

  • Uses the nursing process to deliver high quality care appropriate to the age and educational level of the client.
  • Collects and synthesizes comprehensive health data that are pertinent to the healthcare consumer's health and/or situation.
  • Develops a plan to attain expected outcomes.
  • Analyzes the assessment data to determine diagnoses, problems, and areas of focus for care and treatment, including level of risk.
  • Implements a specified plan: Coordinates care delivery, employs strategies to promote health and a safe environment; uses knowledge of pharmacological, biological and complimentary interventions to restore health and prevent further disability; provides structure and maintains a safe therapeutic recovery oriented environment; uses the therapeutic relationship and counseling interventions to assist health care consumers.
  • Evaluates progress towards attainment of expected outcomes.
  • Responds to chronically mentally ill clients in the community as well as RYC and Trinity locations in need of crises intervention.
  • Coordinates team activities and serves as a resource for unlicensed assistive personnel.

Clinical Excellence

  • Provides assessment, treatment plan development, and monitoring services using evidenced based practice.
  • Assures that all clients receive clinically sound triage/referral and ongoing care for behavioral health needs.
  • Maintains accurate, concise, current clinical documentation. This includes but is not limited to all phone conversations, requests from the client, prescriptions, changes in the clinical conditions and all consultations with providers.
  • Identifies available support systems to meet the needs of each individual client.
  • Assists Robert Young Center clients and families by making contact with community agencies.
  • Establishes and maintains good working relationships with the medical, educational, and social services agencies in the community.
  • Utilizes safe medication administration practices to administer psychotropic medications.

Client Education

  • Utilizes a holistic approach to determine teaching methods appropriate to the individual client.
  • Provides psycho-education to clients and families regarding their illness, medications, and/or treatment plan to supplement information provided by health care professionals.
  • Evaluate the client's understanding of information provided and ability to follow through.

Qualifications

Education: Graduate of an accredited nursing program.

Experience: Two years' experience working with the chronically mentally ill.

License(s)/Certification(s):

  • RN licensure in the state of Illinois.
  • Valid Driver's license.
  • Basic Life Support (BLS) certification.
  • Crisis Prevention Institute (CPI) training within 6 months of hire.
