Salary offers are based on the candidate's equivalent experience and internal equity with other employees within the same job classification. This position is eligible for overtime compensation for all hours worked over 40 in the designated workweek.

As the Health Start Case Manager, you will provide case management services to the highest-risk client population in the Healthy Start Program. High-risk clients include but are not limited to, teen moms, medically fragile infants, pregnant women over the age of 30, and women with a history of substance abuse and/or domestic violence.

We Require

  • Two years of experience working with pregnant women, infants, and/or toddlers in a social service program such as Women, Infants, and Children (WIC), Parents as Teachers, Head Start, or other home visiting programs that involve enrollment, conducting outreach, and/or case management in a health or social service setting

  • Bachelor's degree in Family and Human Services, Social Work, or related field

  • Bilingual Spanish and English-speaking skills

Job Contributions

  • Provide holistic and comprehensive case management services to all clients, including intake assessment, benefit assessment, goal setting, long-term care plan development, weekly case plan development, progress monitoring, individual money management, tenant education, advocacy, and referrals

  • Provide or facilitate crisis intervention during home visits as needed

  • Maintain case files that meet the South Phoenix Healthy Start standard for case files, provide mentoring for case file reviews, and conduct initial monthly file and case reviews with Community Health Workers

  • Develop, track, and monitor each child's health profile. Plan with families or pregnant women on how to identify and meet these health needs. Transport clients to prenatal appointments and encourage them to seek necessary medical services and treatments

  • Serve as a mentor to Community Health Workers. Act as a liaison to other community organizations such as the Department of Child Safety (DCS), mental health agencies, domestic violence shelters, substance abuse agencies, and teen organizations

  • Lead quality assurance efforts for the program. Assess customer satisfaction and program planning by submitting participant cases for periodic review, attending regular case management meetings, presenting various cases to the team for discussion, and surveying program participants to obtain feedback. Recommend program improvements to the management team.

Working Conditions

  • Requires travel to and from job-related locations during the course of a scheduled workday, subject to County policies regarding the use of County vehicles and/or private vehicles on County business

  • Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency

  • CPR and First Aid (adult and infant) will be required within the first six months of employment. Working with minors in high schools requires fingerprinting and background clearance.

  • Must possess or have the ability to obtain, by the time of hire, a valid Arizona driver's license. Mileage reimbursement is available for travel in a personal vehicle as required by this position.

  • All employees, within one month of hire, will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health. Required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chickenpox), and annual Influenza. Some positions require a Hepatitis B vaccine and/or a Tuberculosis test.
