I. Major Responsibilities:

1. Asses the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, medication administration and documentation.
2. Provide appropriate health care and coordinates referrals as prescribed by providers.
3. Maintain documentation required for all primary care clients receiving in-home nursing services.
4. Promote education and adherence to prescribed treatment protocols.
5. Communicates with health care facilities when clients are admitted and plans for discharge to ensure continuity of care.
6. Facilitates wellness education to individuals and offer groups with health and wellness focus.
7. Records all client information within electronic medical record system daily, received by outside healthcare agencies.

II. Position Qualifications:

License/Certification/Education:
Required:
1. 6A: Community LPN: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts

Experience/Skills:
Required:
1. Must be able to pass a CORI background check

Preferred:

1. 2-4 years' experience in medical nursing.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies, including age-specific competencies and their measurements, will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.

III. Physical Demands and Environmental Conditions:

1. Must be able to be in a standing position for extended periods of time.
2. Must be able to hear, understand, and distinguish speech and/or other sounds (e.g., machinery alarms, medicals codes or alarms).
3. Must be able to lift and move objects up to 50 pounds for various needs.
