We Are Hiring:Clinic RN- Senior Health- 1.0 FTE

About Us:

Senior Health Community Health Worker will work closely with Senior Health Center staff providing support to adult patients, particularly those 65 and older, in their homes and in the clinic setting. You will interact with patients/families who are living with varying degrees of cognitive impairment.

Shift/Schedule:This 1.0 FTE commits to 40 hours per week. Typical work hours are M-F 8-4:30


What You Will Do:

The Senior Health Clinic RN utilizes the nursing process to deliver appropriate nursing care and procedures in the home and clinic setting. The Clinic RN performs nursing acts as delegated by a provider. This individual will perform home visits and provide phone support pre-and-post clinic evaluation and use motivational interviewing to support and educate patients/families for optimal management of cognitive decline.



What You Will Need:



  • Registered Nurse in the State of Wisconsin
  • Associate's Degree in Nursing (ADN)
  • Minimum of 2 years of clinical experience in primary care clinic environment or 3-5 years clinical hospital, home care, or community health experience
  • Basic Life Support (BLS) card through the American Heart Associate (AHA), or ability to obtain certification upon hire
