Description

  1. CONDUCT INTAKE ASSESSMENT FOR RESIDENTIAL TREATMENT SERVICES:



  • Answer and respond to all intake related calls.
  • Schedule intake appointments.
  • Interview potential clients and family members to determine admission eligibility.
  • Follow up with all referral sources that include SAPC, DMH, VA, Probation and Parole officers, doctors. psychiatrists and other referral sources.
  • Make recommendations to treatment staff for a client's admission and enrollment.
  • Use the ASAM criteria to develop thorough, written diagnostic assessment that includes client's addiction history, mental health history, medical health history and social functioning to determine medical necessity for treatment.
  • Develop and write up a comprehensive initial treatment plan.
  • Complete and review intake packet with treatment staff.
  • Assist in planning and seeking referrals for client's transition or discharge.



  1. CONDUCT COMMUNITY OUTREACH AND MAINTAIN NETWORK EFFORTS WITH COMMUNITY SUPPORT SERVICES



  • Attend GR/CAL WORKS/ DCFS /SAPC/LAHSA and other service provider meetings.
  • Maintain file on referral resources comprised of community civic groups, the legal system, state and county service groups. i.e., DPSS, and other service providers.
  • Coordinate referral efforts with outpatient, and other outside agencies within the community at large.



  1. PROMOTE TEAM CONCEPT IN IMPLEMENTING TREATMENT GOALS AND



  • Assist Treatment Team in researching and scheduling supportive services such as housing. educational needs. legal services, health and medical services, treatment related referral services, parenting classes, anger management services, domestic violence programs, and others.
  • Facilitate Seeking Safety and other evidenced based support groups.
  • Organize and manage support group topics.
  • Facilitate trauma informed treatment groups,
  • Provide technical assistance with crisis situations.
  • Provide support services in meeting unit's transportation and floor coverage needs.
  • Support and participate in the team's decision-making process.
  • Perform other duties as assigned as assigned by Program manager and/or Director

Qualifications
Education
Bachelors of Psychology (preferred)
Licenses & Certifications
Driver's License (preferred)
TB cleared certificate (preferred)
Motor Vehicle Check (preferred)
Substance Abuse Counselor (preferred)
