Care Management Team Lead - Full-time - OLGMC - Case Management

This job leads the daily operations of all personnel and processes within the care management department. Plans and organizes all care management functions, including training, purchasing, and assisting with the budget process. Leads initiatives to improve processes, meet departmental goals, and maximize efficiency.

Education

Required -

Masters degree in Social Work (MSW). A current RN or LPN license in the state of practice will be accepted in lieu of the degree requirement.

Work Experience

Required -

3 years of utilization and/or case management experience with either a MSW, RN or LPN license,.

Certifications

Preferred -

Current Licensed Practical Nurse (LPN) license in state of practice or Current Registered Nurse (RN) license in state of practice.

Knowledge Skills and Abilities (KSAs)

  • Proficiency in using computers, software, and web-based applications.

  • Effective verbal and written communication skills and ability to present information clearly and professionally.

  • Strong interpersonal skills.

  • Understanding of financial statements sufficient to analyze trends and make recommendations.

  • Working knowledgemanaging within a budget.

  • Deductive reasoning skills and ability to anticipate issues that adversely impact the function of the department and to solve problems.

  • Excellent decision-making, delegating, and technical writing skills.

  • Leadership skills and ability to foster an environment of teamwork, professionalism, customer satisfaction, high performance, and mutual respect of others.

  • Ability to work calmly under pressure, meet project deadlines, work independently, set priorities, and maintain complete confidentiality.

  • Ability to work flexible schedule, such as nights, evenings, weekends, holidays, extended shifts, etc.

Job Duties

  • Guides care management staff in all daily operational performance matters.

  • Uses all available financial and performance management information to monitor and measure the performance of the department as a team as well as the individual performance of team members.

  • Serves as model in customer service matters and educates care management staff in delivering outstanding customer service.

  • Strategizes with leadership in order to achieve departmental and organizational goals.

  • Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style.

  • Performs other related duties as required.
