Description The Substance Use Navigator is an integral team member of a statewide initiative to improve access to Medication for Opioid Use Disorder. The Substance Use Navigator serves as a consistent and knowledgeable resource of Substance Use Disorder (SUD) and Medication for Opioid use Disorder treatment programs and serves as a bridge between the hospital and treatment settings including outpatient care, inpatient care, SUD treatment programs and will connect patients to resources for management of care needs. Utilizing a trauma informed approach, the navigator serves as an advocate to patients in the emergency department and inpatient units who may be interested in treatment services, resources and support. The navigator collaborates with the community resource leaders, physicians, and other members of the health care team, cultivating relationships and serving as a community advocate for the needs of SUD services.
