Substance Use Navigator - Community Outreach

Description

The Substance Use Navigator is an integral team member of a statewide initiative to improve access to Medication for Opioid Use Disorder. The Substance Use Navigator serves as a consistent and knowledgeable resource of Substance Use Disorder (SUD) and Medication for Opioid use Disorder treatment programs and serves as a bridge between the hospital and treatment settings including outpatient care, inpatient care, SUD treatment programs and will connect patients to resources for management of care needs. Utilizing a trauma informed approach, the navigator serves as an advocate to patients in the emergency department and inpatient units who may be interested in treatment services, resources and support. The navigator collaborates with the community resource leaders, physicians, and other members of the health care team, cultivating relationships and serving as a community advocate for the needs of SUD services.

Required qualifications:



  • H.S. Diploma or GED
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year Experience in community social services, case management, or similar setting.


Preferred qualifications:



  • Bachelor's Degree
  • Substance Use Program or medical setting experience
