Join Optum HouseCalls, a field-based care program focused on bringing preventive health assessments directly into members' homes. As an Advanced Practice Clinician, you'll conduct annual in-home health evaluations for Medicare Advantage and other health plan members, helping identify care gaps, educate patients, and collaborate with primary care providers to improve overall health outcomes. This is a non-prescribing role that offers significant autonomy, flexibility, and the opportunity to make a meaningful impact in patients' lives. Position Highlights
  • $10,000 Sign-On Bonus OR $20,000 Student Loan Repayment Bonus available
  • Flexible scheduling with the ability to work between 8:00 AM-8:00 PM across a 7-day week
  • No on-call responsibilities, weekends, or holidays required
  • Additional compensation available for clinicians who are fluent in languages other than English and use those skills during member visits
  • Conduct comprehensive in-home health assessments while collaborating with PCPs to close care gaps and improve outcomes
Required Qualifications
  • Nurse Practitioner: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care; OR Physician Assistant: Current NCCPA certification and state licensure
  • Active, unrestricted state license (or ability to obtain by start date) with no disciplinary actions or practice limitations
  • Active, unrestricted driver's license and reliable transportation for home visits
  • Ability to perform assessments in varied home environments, including sitting, standing, kneeling, and transporting necessary equipment

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
