Join Optum Home & Community Care's HouseCalls team, an innovative, field-based program delivering preventive care directly in members' homes. In this autonomous yet highly collaborative role, you'll conduct annual in-home health assessments for Medicare Advantage and other health plan members, helping identify care gaps, address social determinants of health, and improve overall health outcomes through education and coordination with primary care providers. The program offers strong work-life balance and the opportunity to make a meaningful impact in underserved communities. Position Highlights
  • $20,000 Student Loan Repayment OR $10,000 Sign-On Bonus available for eligible candidates.
  • Flexible scheduling (20-40 hours per week) with provider discretion between 8:00 AM and 8:00 PM across a 7-day workweek.
  • No on-call responsibilities, weekends, or holidays required.
  • Conduct comprehensive in-home assessments focused on preventive care and closing care gaps.
  • Interdisciplinary care model with strong collaboration, patient advocacy, and community resource support.
Required Qualifications
  • Active ANCC or AANP certification (Family, Adult, Geriatric, or Adult-Gerontology Primary Care) for Nurse Practitioners.
  • Active, unrestricted NP license in the state of practice (or ability to obtain before start date).
  • Active and unrestricted driver's license with reliable transportation for home visits.
  • Ability to perform assessments in varied home environments and meet physical requirements of the role.

Compensation for this specialty generally ranges from $109,500 to $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
