Position Title: Care Navigator

Work Location: Remote, GA, Eastern Time Zone

It is a remote position but sometimes travel to the office is necessary for meetings: 1100 Circle 75 Parkway SE, Ste 1100, Atlanta, GA 30339.

Must reside near this location (no more than 45 minutes). We are hoping this is an acceptable commute.

Assignment Duration: 3 Months Potential to extend or convert

Work Schedule: M-F 8-5

Work Arrangement: Remote (occasional onsite meetings required; must reside within 45 minutes of Atlanta office)

Position Summary:

BH Care Management Team Backfill for FTE Care Navigator. Works inpatient and referral queue, outreaches members to offer CM and identify needs, providers resources. Manages a caseload of 90+ members consistently.

Key Responsibilities:



  • Manage member outreach across three queues: Inpatient, Behavioral Health (BH) Referral, and Disease Management.
  • Conduct initial and follow-up outreach to members identified through assigned task lists and queue referrals.
  • Engage members in care management services and, when members agree to participate, coordinate assignment to an appropriate Care Manager.
  • Manage a caseload of 90+ members while completing an average of 20+ member outreach calls per day.
  • Consistently achieve a weekly conversion goal of 5 members enrolled into active Care Management services.
  • Monitor member needs, document outreach activities, and ensure timely follow-up and coordination of care.

1 Ability to manage a 90+ member caseload, identify member needs, coordinate services, and transition members into active care management.
2 Experience conducting high-volume outreach, working BH members and conditions, and engaging members in appropriate healthcare services.
3 Proficiency with healthcare case-management/CRM systems, maintaining task queues, documenting outreach, tracking member status, and managing daily productivity/conversion metrics.
