Title: Health Consultant - Maternal Infant Health Program (MIHP)

Location: Remote/Virtual - Must work and live in the state of Michigan

Purpose: This position will function as a recognized resource/subject matter expert offering specialization and support in the area of public health for pregnant people, infants, families, caregivers, and agency providers. In addition, the incumbent will create educational materials, training modules, and provide support for the state-based Maternal Infant Health Program care coordination portal (MCCP). The individual in this role supports Michigan Department of Health and Human Services (MDHHS).

Duties and Responsibilities:
* Plan, implement,monitor, and evaluate health programs in conjunction with public or private community health agencies, including community-based agencies.
* Examine the feasibility and implications (e.g., fiscal, social, political, legal, and geographic) of policies, programs, and services.
* Develop and interpret rules, policies, and procedures related to health programs and services for implementation by public or privatepopulationhealth agencies, including community-based organizations and health provider agencies.
* Identify, build, andmaintainrelationships with community members and organizations to improve and advocate for community health and resilience needs.
* Ensure that the diversity of individuals and populations is addressed in policies, programs, and services that affect the health of a community.
* Engage community members (e.g., focus groups, talking circles, formal meetings, and key informant interviews) to improve health in a community.
* Facilitate collaboration and foster communication among partners to improve health in a community (e.g., coalition building).
* Design and implement health education and training programs for public and private health agencies and personnel to improve population health.
* Apply and implement strategies for continuous quality improvement in program design and implementation.
* Use public health informatics to develop, implement, evaluate, and improve policies, programs, and services (e.g., integrated data systems, electronic reporting, knowledge management systems, and geographic information systems).
* Establish contacts and meet with public, private, and community-based agencies toassistin designing preventive health programs and procedures.
* Develop educational materials, newsletters, reports, and media to enhance and improve public awareness and confidenceregardinghealth and wellness programs.
* Provide guidance and program developmentassistancefor the state-based Maternal Infant Health Program (MIHP) care coordination portal.
* Collaborate, guide, and consult with MDHHS Maternal Infant Health Program team members and agencies statewide.
* Maintain records and prepare reports and correspondence related to the work.
* Represent the department at external work groups and meetings.
* Represent the unit, section, and divisiononinternal committees and work groups.
* Always Represent the best interests of MDHHS and MPHI.
* Perform all other duties as assigned.

Qualifications/Requirements:

Education:Bachelor of Science in Nursing with current license as a registered nurse in the State of Michigan or Bachelor's degree in Social Work required. Master's degree preferred.

Experience:At least four years of professional experience in maternal and child health. Experience with project management and working with low- income and/or minority populations preferred. Experience working with community capacity building and relations, improving disparities in health outcomes, improving population health via a case management framework, and working with a home visiting program preferred.

* Ability to adapt tonew technologiesand workflow changes.

Work Environment and Physical Requirements:Remote work environment, with occasional travel for in-person meetings. Requires travel across the state. Requires some transporting and transferring of materials to meeting locations, such as displays or boxes of literature.

IMPACT ON PROJECTS, SERVICES AND OPERATIONS: This position has a direct impact on the quality of services provided by MPHI to MDHHS. This position is vital in assisting with the quality improvement efforts of the Maternal Infant Health Program.
