The Primary Care Behavioral Health Licensed Clinical Social Worker (LCSW) provides behavioral health services to adult patients ages 18 and older within UC Davis Health's collaborative, integrated Primary Care model. As an integral member of the multidisciplinary healthcare team, the LCSW partners with Primary Care providers and other healthcare professionals to address the biopsychosocial needs of patients and integrate behavioral health support into their overall care. Using advanced clinical judgment and evidence-based practices, the LCSW provides psychosocial assessment, treatment planning, brief psychotherapy, care coordination, and connection to community resources for patients experiencing depression, anxiety, grief, adjustment-related concerns, and other behavioral health needs commonly addressed in the Primary Care setting. The LCSW advocates for patient self-determination, confidentiality, access to appropriate services and resources, and meaningful participation in decisions affecting their health and well-being while practicing in accordance with the professional ethics and values of social work. This hybrid position offers the opportunity to provide meaningful, patient-centered behavioral health care within a collaborative academic health system while contributing to the continued quality and effectiveness of integrated behavioral health services. On-site work is located near the UC Davis Health hospital campus in Sacramento. The role currently serves the adult population, with the potential to expand services to pediatric patients based on future Department needs. Department Overview Collaborative Care is an integrated behavioral health program that provides individual short-term psychotherapy services to patients within their primary care clinic location. The Social Worker collaborates closely with the patient's primary care provider as well as a consulting psychiatric practitioner. Collaborative Care is supported by the UC Davis Department of Psychiatry and Behavioral Sciences, which is a collaborative team of nearly 80 faculty, staff, residents, fellows, and trainees who work together to provide a comprehensive range of mental and behavioral health services to a diverse socioeconomic patient population throughout the greater Sacramento area.
