The Dialysis Nurse supports Children's Hospital Colorado (CHCO) mission, vision, and core values. This role provides and directs patient care for patients experiencing acute or chronic kidney failure in accordance with applicable scope and standards of practice and policies, values, and mission of the organization. The Dialysis Nurse manages and oversees patients during acute and chronic dialysis treatments to maintain a safe environment, optimize outcomes, and maximize available resources. Additionally, the Dialysis Nurse actively participating in quality improvement initiatives and serves as a subject matter expert in specialty areas.
Duties & Responsibilities
Responsible for the oversight and delivery of direct patient care aligning with established policies, procedures, and state and federal regulations.
Direct patient care includes all dialysis modalities: hemodialysis (HD), peritoneal dialysis (PD), Acute HD and PD. Demonstrates knowledge of Transplant along with other therapies as needed.
Participates in new hire training and orientation activities.
Implements prescribed treatments including Sodium (Na) modeling and Ultrafiltration modeling (when appropriate), ensuring stable treatment therapy.
Assesses patient response to treatment, communicates changes to Nephrology Providers and takes appropriate intervention; actively participates in the pre-evaluation, initiation, monitoring, termination, access care, and post evaluation of patients receiving treatment according to established procedure.
Assesses patient and family readiness and ability to perform dialysis including patient's home environment if necessary. Directs patients in the preventative maintenance of home therapies equipment, including water systems and analyses as appropriate.
Develops and maintains collaborative professional relationships with the interdisciplinary healthcare team to provide culturally competent, age-appropriate patient/family care; collaborates with patients, families, providers, and the multidisciplinary team to facilitate family meetings, establish mutually developed plans of care, and foster shared decision-making; assesses patients' and families' Social Determinants of Health (SDoH) at home or in other community locations, and collaborates with social work and family navigators as needed.
Evaluate and adjust care as needed; accountable for responding to and resolving barriers with providers, patients, family members, and hospital personnel in a timely manner.
Oversees and maintains the patient records including both hardcopies and electronic, for accuracy.
Provides education and coaching to patients and families to improve health literacy and work towards self-management of their health; ensures education needs of patients and families are being met.
Facilitates coordination of patient access to appropriate medical and specialty providers, medication acquisition and management, procedures/tests, appointments, and referrals.
On Call responsibilities for the Acute Dialysis setting, nights and weekends as scheduled.
Minimum Qualifications
Education: Bachelor of Science in Nursing (BSN)
Experience: Three years of nursing experience including two years dialysis, CRRT or Apheresis nursing experience
Equivalency: None
Certification(s): BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire.
Licensure(s): Registered Nurse (RN) license
Salary Information
Pay is dependent on applicant's relevant experience.
Hourly Range: $41.87 to $62.81
