Territory Patient Admissions Coordinator

Compensation:$23-30/hour, depending on experience

Location:Chisago City, MN office - On-site

Position Type:Full-Time


What You Need to Know

The Territory Patient Admission Coordinator establishes and maintains communication with clients, family members, and referral sources. This role is responsible for partnering with agency staff, referral sources, and sales to achieve optimal patient satisfaction & outcomes.

The Territory Patient Admission Coordinator is responsible for the timely acceptance or rejection of referrals, based on criteria set by the agency and facilities, and for obtaining all information and documentation necessary to complete a quality referral.

Territory Patient Admissions Coordinator Qualifications

  • High School diploma or GED
  • Minimum of one year of experience in home care, hospice, or personal care services required
  • Prior use of Electronic Medical Records (EMR), HCHB preferred
  • Experience in dealing with a variety of payors in healthcare, e.g., Medicare, Medicaid, and commercial payers, preferred
  • Medical Back office experience required

Territory Patient Admissions CoordinatorResponsibilities

  • Collaborates and maintains effective communication with the Referral Source, Sales teams, Clients and/or local agency operations to collect needed information to process a quality referral.
  • Documents all referral-related information, actions taken, and follow-up required in the medical record in a timely and accurate manner. As applicable utilizes the agency's software solution to facilitate the referral process by completing required workflow task within the Company's established time frames.
  • Ensures timely attention to referrals received from a variety of platforms, e.g., eFax, telephone, or referral portal/Hospital EMR portal.
  • Where applicable, reviews Eligibility Alerts and Responses for Medicare Clients.
  • Requests, H&P, Discharge Summary, and other documents needed for ICD 10 coding by the CPOCC team, if not received with the referral information.
  • Completes a "Follow Up" call to the Client/Caregiver/Referral Source to collect or confirm information. Example: confirms out of pocket expense, clarify teachable caregiver, and collect missing demographic and physician information.
