THE POSITION

Are you familiar with community resources, benefits, and aging-related programs? Northumberland County Area Agency on Aging is seeking a compassionate and knowledgeable Aging Care Manager 2 to help older individuals and their families navigate care options, access vital services, and maintain independence with dignity. DESCRIPTION OF WORK

In this role, you will support older adults by assessing needs, developing care plans, and helping individuals access important services. Your work will focus on improving quality of life through coordination, evaluation, and collaboration with service providers. As a Aging Care Manager 2, you will perform the following duties:

  • Care Planning: Develop care plans based on evaluations of daily living needs, health status, and available supports
  • Service Coordination: Refer individuals to agencies and programs that can provide additional assistance
  • Client Support: Help older adults understand and use services such as Social Security and Medicare
  • Case Evaluation: Review case information and prepare reports, histories, and documentation
  • Agency Collaboration: Work with hospitals, clinics, and community resources to support client needs
  • Supervisory Duties: Oversee staff work when required and assist with operational needs

Work Schedule and Additional Information:

  • Full-time employment
  • Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with a 30-minute lunch.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Minimum Experience and Training Requirements:

  • Successful completion of the County Social Casework Intern program; or
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; or
  • Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
  • An equivalent combination of experience and training.

  • Hiring preference for this vacancy may be given to candidates who live within Northumberland County. If no eligible candidates who live within Northumberland County apply for this position, candidates who reside in other counties may be considered.

    • Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
