THE POSITION

Make a difference every day as a Community Health Nurse with the Department of Health. This role allows you to support local communities by promoting strong public health practices. DESCRIPTION OF WORK

This position strengthens public health efforts by assessing needs, planning services, and carrying out health initiatives throughout the community. It supports education, clinical care, and partnerships that improve health outcomes. As a Community Health Nurse, you will perform the following duties:

  • Public Health Assessment: Analyze county health data to guide programs and activities
  • Community Outreach: Link Department of Health resources with local partners to support health goals
  • Program Oversight: Monitor health programs and use public health information systems to improve outcomes
  • Education Support: Deliver health education programs on key public health topics
  • Clinical Services: Provide immunizations and disease prevention services following established guidelines
  • Disease Investigation: Conduct surveillance and investigations to protect community health

Work Schedule and Additional Information:

  • Full-time employment, 37.5 hours per week
  • Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
  • Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays.
  • Telework: You will not have the option to telework in this position.
  • Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS

Special Requirements:

  • This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
  • If you possess an active temporary practice permit, you must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing.

  • You must be able to perform essential job functions.

Legal Requirements:

  • You must pass a background investigation.
  • This position falls under the provisions of the Child Protective Services Law.
    • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
