As an active participant in the primary care clinic, the LPN engages in a collaborative process with members of a multidisciplinary team, which may include a physician, nurse practitioner, clinic care manager, social worker, clinic manager, medical assistant, and patient care coordinator. The team assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet a patient's health needs through communication and utilization of available resources to promote efficient, high-quality outcomes.

The LPN provides quality nursing care to patients, supports the providers for whom they work, delegates tasks to clinic medical assistants, and oversees health, safety, and quality efforts within the clinic setting.

The LPN collaborates with the primary care provider and other members of the care team by providing routine nursing care and patient education to ensure that patient needs are met. In addition, the LPN oversees efforts within the clinic to promote quality, safety, and efficiency.

Primary Responsibilities:
As part of a multidisciplinary care team, the clinic LPN will:

  • Provide direct nursing care in collaboration with an ordering provider, within the scope of LPN licensure
  • Utilize clinical decision-making to execute standing orders to meet patient needs and improve clinic efficiency
  • Evaluate communication from patients via telephone and portal messages to determine priority, escalate urgent needs to the appropriate member of the care team, and assign routine communications to medical assistants
  • Delegate appropriate responsibilities to medical assistants to promote patient safety while improving efficiency
  • Serve as a clinical resource to medical assistants and provide education to non-licensed clinical staff
  • Assist with complex procedures
  • Act as a liaison between clinic providers and unlicensed staff to promote collaboration and understanding
  • Manage patient medication refill requests
  • Oversee clinical health and safety efforts, including but not limited to laboratory quality control monitoring, medication storage and administration, and infection control initiatives
  • Assist with the ordering and receiving of medical supplies and medications as needed
  • Demonstrate leadership and accountability to support a safe, high-quality workplace culture
  • Perform clinic and administrative tasks as directed by the clinic manager

Required Qualifications:

  • Current LPN License
  • Current CPR certification
  • Experience in the healthcare industry - clinical experience
  • Computer experience
  • Thorough knowledge of Regulatory Activities to include OSHA /HIPAA compliance

Preferred Qualifications:

  • EMR training

Physical Demands /Working Conditions:

  • Ability to stand for long periods
  • Extensive walking
  • Must be able to lift up to 20 pounds
  • Ability to bend and sit

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $20.38 to $36.44 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
