Job Summary

The Community Health Worker (CHW) will support outreach and engagement for Drexel HOPE's mobile medical unit. This role is ideal for someone who enjoys working directly in the community, building relationships with local organizations and residents, and helping connect individuals to health and social services.
The CHW will play a key role in strengthening community partnerships, supporting mobile clinic operations, and helping to identify locations where the mobile unit can best serve community members. This position works closely with an interdisciplinary care team to promote harm reduction, improve access to care, and support linkage to ongoing services.

This position is grant-funded; employment is contingent upon the continued availability of those funds.

Essential Functions



  • Serve as a liaison between Drexel HOPE and community organizations, cultivating and maintaining partnerships with local stakeholders.
  • Conduct community outreach including door-to-door engagement to increase awareness of Drexel HOPE services and identify locations
  • Assist with identifying and coordinating future mobile clinic locations in collaboration with community partners.
  • Provide harm reduction education, including but not limited to opioid overdose prevention and naloxone training, to clients and community members.
  • Support client engagement through EHR registration, and appointment scheduling.
  • Conduct intake assessments using a trauma-informed, strengths-based approach.
  • Assist clients with care navigation and coordination, including linkage to primary care, MOUD, behavioral health, mental health, and other supportive services.
  • Advocate for clients and help reduce barriers to accessing health and social services.
  • Participate in grant-required documentation, data collection, and data entry for program reporting.
  • Represent Drexel HOPE at community meetings, outreach events, and trainings.
  • Assist with transportation and coordination of outreach supplies for the mobile clinic when needed.
  • Travel to mobile clinic sites and community locations as required.
  • Perform other related duties as assigned.


Required Qualifications



  • Minimum of a HS Diploma or GED
  • Minimum of 2 years work related experience in social services


Preferred Qualifications



  • Community Health Worker (CHW) certification, Certified Peer Recovery Specialist (CPRS) certification, or related training in community health, peer support, harm reduction, or patient navigation.
  • Prior experience conducting community outreach or engagement, particularly with underserved or marginalized populations.
  • Familiarity with harm reduction principles and overdose prevention education.
  • Experience working with individuals affected by substance use, homelessness, infectious diseases, or behavioral health conditions.
  • Knowledge of local community resources and social service systems in Philadelphia.
  • Comfort working in nontraditional settings such as community spaces, outreach environments, or mobile clinics.
  • Valid driver's license and ability to assist with transportation of outreach supplies and travel to community locations.

Additional Information

This is a Part-Time per diem position classified as Non-Exempt. The offered pay is based on the successful candidate's qualifications and experience, department budget, and an internal equity review.
