Summary:
The Care Management Social Worker I role is responsible for the ongoing assessment of the biopsychosocial-spiritual status of the patient and family/supports. The social worker will work collaboratively with the interdisciplinary team members, both internal and external to the organization to facilitate discussions regarding discharge options as they relate to complex situations, facility placement, or community resources in the context of the patient's health insurance, available support, location and other eligibility factors.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

  1. Education: Master of Social Work Degree from an accredited school required.
  2. License/Certifications: Licensed by the State of Maine as LMSW or LMSW-CC required.
  3. Experience: Two years of experience in a hospital setting preferred.
  4. Additional Skills/Requirements Required: N/A
