ESSENTIAL FUNCTIONS/DUTIES AND RESPONSIBILITIES:



  • Provides coordination of client care, evaluates and ensure continuity and quality of care
  • Conducts initial and quarterly assessments for home care clients both in-house and off campus.
  • Ensures staff understanding of, and compliance with agency policies and procedures.
  • Reviews documentation and performs In-home supervisor visits
  • Communicates with physicians, staff, family and clients
  • Completes chart audit and Discharge summaries for Home Care clients
  • Back up to the Clinic Nurse Manager for assistance and coverage
  • Performs direct and individualized nursing care to an assigned group of clients.
  • Participates in planning, implementing and evaluating care plans and nursing care
  • Performs and/or assists with treatments, therapies and procedures
  • Administers and records oral, intramuscular and subcutaneous medications in accordance with home care services policies, procedures and physician orders
  • Writes initial nursing assessments upon admission, assesses clients conditions and assists with the development of the individual care plans for clients
  • Documents observations, treatments, client response to treatment and nursing care. Completes periodic assessments
  • Assists in the development, updating of care plans, and IDCP process
  • Gives shift report, documents on 24-hour report, processes physician orders, and assists with orientation of new employees
  • Participates in continuing education, staff meetings, and other required in-service training
  • Supports and adheres to administrative and nursing policies and procedures
  • Knows and is able to initiate fire and disaster plans. Recognizes and responds to client emergencies with appropriate measures
  • Utilizes resources, materials and equipment in a safe and efficient manner
  • Demonstrates dependability by being punctual; maintains scheduled attendance
  • Supports and enforces safety and infection control policies and procedures
  • Conforms to facility dress code
  • Performs routine housekeeping duties including straightening and maintaining service areas and resident rooms
  • Cleans, disinfects and maintains resident care equipment as assigned
  • Completes all other duties as assigned by supervisor

Qualifications

Education required:

Current license to practice as a Registered Nurse or Licensed Practical Nurse in North Carolina.

Experience required:

Geriatric experience preferred.

Specific skills, knowledge and abilities required:

Knowledge of the federal and state guidelines for home care and long term senior care. An understanding of nursing care and a knowledge of human needs and emotions. Ability to communicate effectively with others verbally and in writing.
