Primary Responsibilities:
Medication Adherence & Quality Measures Support
*Identify patients at risk for medication non-adherence using centralized PHM reports, payer files, registries, refill history, and EHR tools.
*Primarily support Medicare Advantage Star medication adherence measures while addressing adherence gaps across all populations.
*Conduct patient outreach (phone, portal, text as appropriate) to address late refills, access barriers, and affordability concerns.
*Educate patients on refill timing, dosing schedules, and the importance of adherence, within technician scope and under pharmacist guidance.
*Support adherence interventions such as refill synchronization, automatic refills, reminder outreach, and coordination with retail, specialty, and mail-order pharmacies.
Care Coordination, Access & Affordability
*Support patients across all lines of business (Medicare Advantage, Medicaid, Commercial) with pharmacy-related access and affordability barriers that impact medication adherence.
*Assist with prior authorization follow-up, formulary alternatives, refill coordination, pharmacy transfers, and medication delivery options.
*Support cost-reduction strategies such as generic alternatives, preferred formulary options, pharmacy optimization, manufacturer assistance referrals, and coordination with affordability or financial navigation teams.
*Coordinate with retail, specialty, and mail-order pharmacies to resolve delays, coverage issues, or refill discrepancies.
*Escalate clinical questions, adverse effects, or therapy concern promptly to a pharmacist or licensed clinician.
Documentation, Reporting & Quality Tracking
*Document all patient interactions, outreach attempts, and outcomes accurately in the EHR, PHM platforms, and/or adherence tracking tools.
*Maintain centralized adherence worklists and ensure timely completion of assigned interventions.
*Track and support closure of adherence-related quality gaps tied to Medicare Advantage Star Ratings.
*Assist pharmacists with preparation of reports, audits, and payer requests related to adherence performance.
Team Collaboration
*Work closely with pharmacists, medication access coordinators, care managers, and providers to support patient-centered medication management.
*Participate in team huddles or meetings related to adherence initiatives and population health priorities.
*Support workflow optimization and continuous improvement initiatives related to medication adherence programs.

Qualification Requirements:

  • High School Diploma or equivalent.

  • Pharmacy Technician license or Technician Certification (CPhT).

  • 2+ years of experience, preferably in a hospital setting.

  • Familiarity with medication refill processes and pharmacy workflows.

  • Strong communication skills and comfort engaging patients by phone.

  • Basic computer proficiency and experience with electronic health records (EHRs) or pharmacy systems.

Preferred Skills:

  • Health Plan / Medicare Advantage experience strongly preferred

  • Background in Medication Adherence from a Health Plan or Hospital strongly preferred.

  • Experience working in a centralized population health or value-based care model.

  • Experience supporting Medicare Advantage populations and quality initiatives.

  • Experience assisting patients with medication access, affordability, and coverage issues.

  • Working knowledge of Medicare Star Ratings and adherence methodologies (e.g., Proportion of Days Covered [PDC]).

  • Experience using payer reports, registries, or quality dashboards.

  • Bilingual skills.

Additional Knowledge, Skills and Abilities:

  • Shows knowledge of the pharmacology of drugs and the legal aspects of dispensing.

  • Familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.



Schedule and Working Model:

  • Full-time, Monday through Friday, standard business hours.

  • Remote with occasional in-office requirements at Assembly Row in Somerville, MA.
