Job Description:
- Program Specialist shall communicate with or Program Specialists when reentrants overdose in community and are returned for detox to determine eligibility and appropriateness for .
- Program Specialist shall assist with referrals in community, including setting up and scheduling appointments for medication and treatment services.
- Program Specialist shall ensure assigned individuals receive needed services, benefits, and supports in accordance with treatment plans.
- Program Specialist shall track and update a spreadsheet and or database for all individuals released on .
- Program Specialist shall attend weekly Vivitrol conference calls and or meetings as required.
- Program Specialist shall document all contacts and services in appropriate systems such as Captor and Sapphire.
- Program Specialist shall attend all required trainings.
- Program Specialist shall meet with reentrants who have positive urine drug screens to assess needs and refer to appropriate treatment.
- Program Specialist shall complete required GPRA documentation including initial, six month follow up, and discharge.
- Program Specialist shall meet with individuals in correctional facilities, community settings, parole offices, and or treatment provider locations as necessary.
- Program Specialist shall communicate treatment planning information to center staff, facility staff, and or parole staff as appropriate.
- Program Specialist shall adhere to and AOD treatment frameworks, policies, and procedures of Department of Corrections.
- Program Specialist shall provide social work, counseling, and case management services to enhance individuals' social functioning and support successful social, economic, and physical adjustment.
- Program Specialist shall perform duties in accordance with applicable rules, regulations, policies, and procedures under supervision of an administrative supervisor.
- Program Specialist shall assess individuals' psychosocial strengths, needs, eligibility for services, and barriers to treatment.
- Program Specialist shall counsel individuals using a variety of evidence based treatment modalities.
- Program Specialist shall present psychosocial assessments and family histories in multidisciplinary treatment team meetings.
Requirements:
- A bachelor's degree in social work or a related human services field is required.
- Program Specialist shall demonstrate knowledge of professional social work and casework principles, individual and group behavior, counseling techniques and interventions, commonly occurring physical and mental illnesses and implications, social, economic, health, habilitative, and rehabilitative resources, HIPAA requirements and state confidentiality laws, and applicable rules, regulations, policies, and procedures related to social work services and programs.
- Program Specialist shall demonstrate proficiency in Microsoft Excel and ability to establish and maintain effective working relationships, communicate effectively in writing, and communicate effectively verbally.
