Role and Responsibilities:

  • The incumbent will be assigned to the Office of Long-Term Living (OLTL) Chief of Staff Office, under the direct supervision of the OLTL Chief of Staff. Pennsylvania's Community Health Choices (CHC) managed care plans are responsible for providing long-term care supports and services to persons living in the community and those residing in a nursing home. OLTL requires each CHC managed care organization (MCO) to help identify candidates for transitioning from a nursing home to the community setting of their choice. OLTL uses Money Follows the Person (MFP) funds from the federal government to operate several initiatives that help OLTL participants safely and successfully transition from nursing facilities to the community setting of their choice. In addition to OLTL's activities, each CHC MCO employs their own strategies for nursing home transition. The incumbent will work to coordinate these activities and promote best practices in the transitioning of persons to the community.
  • Track in an organized manner all OLTL projects that receive MFP funding.
  • Draft all summary reports for end of year reviews regarding OLTL MFP project status, including budgets and any submissions for continuation of funding for a project the following year.
  • Act as liaison between OLTL MFP project leads/staff and the Client (Client) MFP Coordinator.
  • Act as liaison between budget/contract leads and OLTL MFP project leads.
  • Schedule and facilitate meetings to be held on a regular schedule and as needed to review budget requests and budget updates, including funds spent and travel costs if applicable.
  • Review and submit all annual reports on OLTL MFP projects to the OLTL Deputy Secretary for review and approval.
  • Submit all approved annual reports on OLTL MFP projects to the Client MFP Coordinator.
  • Review and submit all new project proposals to the OLTL Deputy Secretary for review and approval
  • Submit all approved new OLTL MFP project proposals to the Client MFP Coordinator.
  • Participate in all meetings/discussions regarding project needs/status and any new proposals for request of MFP funding.
  • Answer inquiries from internal and external parties regarding MFP projects/funds.
  • Consult with others internal and external to OLTL and Client as necessary to ensure each MFP project meets its goals.
  • Work with supervisors in OLTL to develop job descriptions for new positions needed in OLTL's bureaus to do MFP work.
  • Research the work of other states and apply research findings to recommend best practices and new activities to support MFP projects in OLTL.
  • Evaluate new OLTL initiatives for the potential of utilizing MFP funds.
  • Assist subject matter experts in OLTL's bureaus with setting project milestones and tracking the progression of projects to meet timelines associated with MFP projects.
  • Identify and promote best practices for OLTL MFP projects.
  • Other related duties as assigned.
