Pay Transparency:

The above reflects the anticipated hourly wage range for this position if hired to work in New Jersey.

The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Participate as a member of the multidisciplinary treatment team, assisting in case management responsibilities of patient care in the care of Child and Adolescent Outpatient Department Team.

Qualifications:

Required:

  • MSW with LSW, or MA in Counseling with LAC
  • Minimum of one year experience in patient care or medical office management with direct patient interactions
  • Skills Training (preferred) experience with EMR and electronic scheduling systems, experience working with a clinical team in a patient care setting.

Scheduling Requirements:

  • Full Time
  • 9:00am-5:00pm (there will be a need for at least 1-2 evenings until 6pm)
  • Days to work: Monday to Friday

Essential Functions:

  • Facilitate adherence in Psychiatrist/APN scheduling, case management, documentation in Electronic Medical Record (EMR), collateral contacts for patient care purposes, consumer education, treatment planning, coordination of referrals and intake evaluation as necessary.
  • Conduct case management and therapy sessions directly with patients and families.
  • Facilitate group therapy as needed. Maintain and monitor all case management cases in department.
  • Perform intake evaluations, including warm handoffs between programs, clinical case management and treatment planning with consumers that experience psychiatric disorders.
  • Maintain records of all interdepartmental transfers, Psychiatrist/APN case lists, and community resources.
  • Record and maintain closed case list and manage transfer to Medical Records Department.
  • Conduct reminder calls for Psychiatrist/APN appointments.
  • All clinical documentation is completed with compliance in the requirements set by the NJ Division of Mental Health Services and DNV.
  • Participates in staff training and continuing education.
  • Takes an active role in Performance Improvement projects.
  • Is responsible for reporting identified safety issues such as hazardous environments and medical errors as well as child abuse and neglect reporting to DCPP.
  • Other duties as assigned.
