Job Responsibilities

  • Determine Eligibility: Through communication with the family, school, clinical staff, and review of EPIC documentation, the education team member will determine patient eligibility for HSP services.
  • In collaboration with the HSP manager and educational coordinator(s), evaluate all pertinent patient information to determine appropriate type and level of services (such as 1:1 sessions, small group sessions, family advocacy, school re-entry support, consultation).
  • Plan and Provide Educational Instruction: Coordinate with HSP team members to obtain required authorization documents from family in order to communicate with school staff and to exchange records and reports.
  • Assess student's current level of functioning across all developmental domains (cognition, communication, fine/gross-motor, social-emotional) through communication with patient's school, family, multi-disciplinary team members, and use of formal and informal assessment tools.
  • Plan and design goal-based, creative, multi-modal educational program that aligns with patient's educational needs and school assignments/curriculum when possible.
  • Provide daily group or individual educational services in accordance with established schedule and patient condition or status.
  • Maintain daily charting on progress towards goals and attendance utilizing organizations data tools.
  • Provide Education and Advocacy/Act as Liaison: Establish communication with patient's home school teaching team to determine current educational programming and supports.
  • Maintain ongoing contact with school to facilitate continued academic success and to provide updates on progress during hospitalization.
  • Collaborate with family and HSP education coordinator to support smooth transition back to school following discharge.
  • Multidisciplinary Team / Communication: Document educational services provided and recommendations, as appropriate, in the patient's medical record.
  • Integrate multidisciplinary team goals for the patient into the educational program and individual interactions when appropriate.
  • Interpret educational program and provide multidisciplinary team with pertinent educational information by participating in unit/psychosocial/therapeutic rounds, or discharge planning meetings as appropriate.
  • Recommend consults/referrals when student needs are beyond the scope of the school program.


Education Requirement

  • Required Education: Bachelor's Degree in Special Education, Elementary Education, or Secondary Education.
  • Preferred Education: N/A


Required Certification & Licensure

  • PA Teacher Certification from Pennsylvania Department of Education upon hire.


Preferred Certification & Licensure

  • Special Education PA Teacher Certification from Pennsylvania Department of Education.


Knowledge, Skills, Abilities

  • Required Skills & Experience:

    • Ability to provide appropriate individualized academic assessment and instruction 1:1 and in a small group format to children of varied racial, ethnic, cultural, religious, and social backgrounds.
    • Ability to work collaboratively in a multi-disciplinary, healthcare setting.
    • Ability to demonstrate knowledge and effective use of behavioral support strategies.
    • Ability to adapt programs, equipment, and materials to suit the individual needs of children with a variety of medical conditions and learning differences.
    • Ability to demonstrate flexibility in scheduling and delivery of educational programming.
    • Ability to understand the effects of illness, physical disability, and hospitalization on children, family members, and school personnel.
    • Ability to understand and develop ways to alleviate stress in children.
    • Strong organizational skills with the ability to prioritize and work on multiple projects and programs.
    • Ability to communicate effectively, verbally and in writing, with the broadest range of individuals including children, families, the general public, and professionals.
    • Ability to work in an intense and stressful environment.
    • Knowledge of hospital and community resources.
    • Respiratory FIT Testing.


  • Preferred Skills & Experience:

    • At least one (1) year teaching experience with the appropriate population (e.g., elementary, secondary, special education).
