Description
The Clinical Pharmacy Specialist - Multidisciplinary Ambulatory Care provides advanced clinical pharmacy services in support of specialty ambulatory practices across the health system. Working collaboratively with specialty providers, the pharmacist delivers medication therapy management, therapeutic optimization, patient education, and monitoring for complex disease states within specialties including, but not limited to, Cardiology, Nephrology, Pulmonology, Psychiatry, Urology, and Women's Health. This role also supports the UH Culture Callback Program by performing clinical review of positive microbiology results for patients discharged from the emergency department, ensuring appropriate antimicrobial therapy, and communication with patients to promote safe transitions of care and optimal clinical outcomes. A Brief Overview The Clinical Pharmacy Specialist provides a wide range of services in their area of expertise. These services may include, but are not limited to, rounding with physicians and other healthcare providers to drive evidence based and institution guideline directed medication therapy management, active participation in the training of pharmacy students and pharmacy residents, assist with policy and guideline development, and active participation in local and system level committees and workgroups in their area of expertise.
