NP Provider Hospice & Palliative Care (Pediatric)

This field-based position will cover the following locations: Manhattan, Bronx, Brooklyn, Queens, and Long Island.

MJHS is a large not-for-profit health system in the Greater New York area. Our range of health services include home care, hospice and palliative care for adults and children, rehabilitation and nursing care at Menorah and Isabella Centers, and the research based MJHS Institute for Innovation and Palliative Care. • He/she will provide NP-appropriate service to patients enrolled in hospice and may also assist in staffing pediatric palliative care consultation services that function as a program of MJHS Hospice and Palliative Care
• He/she will work in the community environment, and may, on occasion, provide consultation in the long-term care environment or hospital.
• In all settings, he/she will provide in-person visits or telehealth visits, as appropriate.
• Providing comfort care consistent with best practices, including history-taking, appropriate examination, formulation and care planning, treatment recommendation or implementation, communication, and documentation
• In the role as hospice NP, he/she will coordinate with the team physician and manager and will be responsible for face-to-face evaluations for hospice eligibility, and the assessment and management of hospice patients in need of exigent medical care
• He/she will also contribute to the training of the hospice team, providing support for the interdisciplinary team, as needed
• He/she will also help build a teaching curriculum on pediatric aspects of hospice care that will be of use to our hospice workers who are engaged in pediatric care through cross coverage for when young adolescents are incorporated into adult hospice teams
• In providing both direct care and support to other clinicians, the NP will use an evidence-based approach to management and to the use of materials and policies/procedures
• Other activities related to quality improvement projects may also be required, as appropriate

* Hospice & Palliative Care certification strongly encouraged by year 2 of hire.
