The Diabetes Coordinator Registered Nurse (RN) coordinates diabetes-related care, services, and education throughout the continuum of care. Working without supervision, this role analyzes diabetes-related data, participates in research activities, and reviews program outcomes to ensure safety in the hospital and during transitions of care. This role will provide both comprehensive diabetes management services to patients as well as consultative services to interprofessional team members. The Diabetes Coordinator / Registered Nurse is a subject matter experts on situations that require a deeper understanding of diabetes management and glycemic control.

Essential Job Function
  • Analyzes diabetes-related data to identify trends, patterns, and areas for improvement in diabetes management across the organization.
  • Collaborates with interprofessional teams to prevent patient harm and improve safety through consultation, effective communication, creating evidence-based practice recommendations and policies, and developing education for healthcare team member.
  • Participates in research activities related to diabetes management and treatment as a means to contribute to the advancement of evidence-based practice in the field and stays up to date with the latest developments in diabetes care.
  • Reconciles information from various sources to ensure accurate and comprehensive diabetes-related care and documentation.
  • Reviews program outcomes and performance indicators to assess the effectiveness of diabetes care initiatives.
  • Performs other duties as assigned.

Education
  • Required: Bachelors Degree in NursingPreferred: Masters Degree in Nursing

Experience
  • Required: 3 years as a Registered Nurse and 1 year in a clinical specialty as defined by ANCC or national nursing organizations

Licensure/Certification/Listing
  • Required:Registered Nurse license in North Carolina or a Compact stateNursing certification in specialty area within 1 yr of hire.BLS (CPR)-American Red Cross or AHA Healthcare Provider
