Summary Job Description: Job Title: Care Transitions Navigator Full Time 1st Shift Position 4/10 hour Shifts 0800 - 1830; weekends required; NO ON CALL The Behavioral Health Unit (BHU) is a secured, therapeutic inpatient setting that provides short-term, intensive psychiatric care to patients experiencing acute mental health crises. The unit is staffed by a multidisciplinary team, including psychiatrists, psychiatric nurses, social workers, therapists, and behavioral health technicians, who collaborate to develop individualized treatment plans. Job Description: The Care Transitions Navigator is a certified position that includes hands on interaction with patients who have complex needs including but not limited to psychosocial, psychiatric, and/or substance use issues presenting to Baptist Health Richmond. Referrals and support could include but are not limited to: Mental health treatment, substance abuse treatment, medication assistance, housing assistance, food security, childcare, grief support, community support groups, financial assistance, PCP/Home Health/Dental services, psychiatric and/or substance abuse long term care facilities, legal assistance, etc.
- Responsible for facilitating appropriate ongoing referrals for treatment and/or community-based services/needs to best serve the psychosocial and/or behavioral health needs of the patient.
- Through collaboration with physicians, providers, Behavioral Health Therapy Navigators and nursing staff, ensures that the patient has an effective transition from hospital to post-hospital care through planning, relaying of information, and referral to appropriate community agencies.
- Provides guidance and/or resources for any other non-medical problem the patient may have that may hinder his/her rate of recovery, emotional and therapeutic support, and crisis intervention services.
- Serves as a resource for continuity of care options after discharge and assists in the coordination and referral of such options as needed.
- Works within a high-volume, high-energy department so the Care Transitions Navigator working within this environment must be intuitive, creative, and have the ability to multitask.
- Take periodic on call coverage and respond in timely manner within (1) hour.
Minimum Education, Experience, Training, and Licensures/Certifications/Registrations Required:
- Master's degree in social work with a current KY License as a Certified Social Worker (CSW) or current KY License as a Licensed Clinical Social Worker (LCSW) required.
- For the CSW, supervision hours must be completed within 3 years of hire, and licensure must be obtained within 8 months of completing the supervision hours.
- Master's degree in counseling with associated licensure (LPCA, LPCC) may be considered in place of the social work degree/licensure.
- Professionals must good standing with Kentucky Boards of Social Work and Professional Counseling.
- Minimum of 2 years clinical experience preferred.
- Preferably has experience completing psychiatric and/or substance use disorder consultations within this population. Knowledge of the various levels of care and community resources.
