Position summary: The Nurse Navigator for Triage, TNN, is a licensed professional registered nurse who serves as the clinical point of contact for oncology patients, and families,

The TNN provides telephonic and electronic triage, assesses clinical symptoms, prioritizes urgency, coordinates care transitions, and facilitates timely connection to oncology services,

Demonstrates exceptional assessment, critical thinking skills, and customer service skills,

The role integrates expert oncology knowledge with navigation principles to ensure safe, patient-centered, and efficient access to care and symptom management,

Essential Functions

- Clinical Triage & Care Coordination

- Perform comprehensive assessment of patient symptoms via phone, electronic message, or referral platform to determine acuity and direct the next steps of care,

- Applies critical reasoning, utilizing evidence-based triage protocols and astute clinical judgment to differentiate urgent vs,

routine needs and provide symptom management guidance,

- Maintain open and consistent communication with the care team, including physicians, advanced practice providers APPs, and nurse clinicians, to ensure continuity, clarity, and safety in patient care,

- Collaborate with the care team to arrange same-day or next-day appointments for urgent cases, and schedule follow-up visits as appropriate or referral to emergency services as needed,

- Provides clinical advice, medication guidance, and patient education within nursing scope of practice, related to cancer treatments,

- Document all interactions in the electronic medical record, Epic and ensures appropriate escalation and follow-through,

Oncology Navigation

- Collaborate with the multidisciplinary teams to address urgent, and emergent needs,

- Transitions patients to appropriate services, using the nursing process to develop a plan of care to address emergent needs in communication with the sites care team,

- Clinical & Technical Expertise

- Proficient in oncology nursing practice, symptom management, and chemotherapy-biotherapy principles- ONS standards,

- Competent in telephonic triage, Epic documentation, and electronic communication tools,

- Utilize critical thinking and evidence-based practice in patient assessment and problem solving,

- Participates in ongoing evaluation and revision of triage protocols and workflows in collaboration with the Access Center Navigation leadership,

- Participates in quality improvement initiatives, data tracking, and outcome reporting, e,g, response times, call volumes, disposition outcomes,

- Educates patients and caregivers on, treatment expectations, and self-management of side effects,

- Attends staff meetings, in-services, and continuing education to maintain current oncology and triage knowledge,

- Participates in own professional development by maintaining required competencies, identifying learning needs and seeking educational offerings,

Job Qualifications- Education- Training- Certification-Licensure Comments

Education: BSN required, MSN Preferred

- Minimum of three years of nursing experience required,

Minimum of 2 years in direct oncology care, triage experience preferred

- One year of customer service experience in a healthcare setting preferred

- Licenses by the NJ State Board of Nursing as a Registered Nurse BLS provider required

- Certification required within three years of employment
