Norris' Group Care Programs provide treatment, educational, and therapeutic recreational services to youth ages 8 through 18 and their families. Youth receive services related to mental health, emotional behavioral disorders, problematic sexualized behaviors, AODA, and trauma, including physical, sexual, emotional abuse or neglect. Norris is seeking an organized and energetic individual to join our Clinical team as a Group Care Case Manager at our Mukwonago campus! This is an exciting position that offers a variety of opportunities to make a difference in the lifes of our youth. This role will oversee cases for both residential and shelter care programs.
Under the supervision of the Director of Clinical Services (DCS), the Group Care Case Manager (GCCM) is responsible for providing case management services to youth, and coordinates the admissions process for all program referrals to include residential and other programs assigned. Additionally, the GCCM assists program leaders, clinicians, and the Norris Academy employees to ensure a therapeutic environment is maintained on the hall and in the school setting, personalized treatment needs are addressed, trauma informed practices are utilized, and barrier needs of youth and families are attended to in order to promote sustainable results in the areas of safety, permanency, and well-being.
*The GCCM position requires the individual is able to transport youth and/or families served in a Norris-owned or personal vehicle when needed.
*The GCCM position requires being able to work a flexible schedule that is subject to change to meet the program's evolving needs at any time, and includes some evening hours, weekends and/or holidays. One late night per week is required.
Service Planning & Teaming
- Facilitates regular communication and collaboration among school employees, clinicians, family members, case workers, the courts, probation, psychiatrist and other key team members for treatment planning and quality care (i.e., prepares for and attends med reviews, identifies need and organizes staffing meetings, attends team meetings, as requested, etc.).
- Knows, implements and provides input on each youth's treatment, individual, and guidance plans.
- Is the point of contact for the placing Norris regarding treatment planning, overall progress of the youth and identifying extraordinary needs/resources beyond standard contract requirements.
- Ensures collaboration of safety and service planning both internally and externally so that youth have opportunities to achieve goals effectively and timely.
- Works closely with the Health Liaison and program leaders to ensure youth are receiving appropriate medical, vision, dental, mental health, educational services, and other identified needs.
- Continuously assesses youth progress, and clearly articulates in accordance with the permanency goal.
- Prepares for and participates in and/or facilitates team meetings/staffings with placing agencies.
- Collaborates frequently with the Norris Academy to assess and understand the educational needs of the youth, especially regarding IEP plans.
Youth Services
- Actively interacts with the youth, providing and modeling direct service and effective supervision.
- Attends and/or provides transportation for youth appointments including, but not limited to: medical/dental, mental health, school, family interactions, team meetings, court appointments, and others as needed.
- Assists hall leadership to coordinate routine group and recreational activities while ensuring youth safety.
- Ensures case staffings are coordinated and scheduled per internal protocol and placing agency contracts.
- Assists shift supervisors with making purchases for youth/the hall, like clothing, medication, food, etc. as needed.
- Secondary Medication Passer: May periodically be responsible for packing medications for off-campus administration or administering medications to youth while off-campus (hall activities, while in transit to or attending team meetings, court hearings, etc.)
- Other duties as assigned for the overall purpose of the program.
Referrals and Admission
- Be the point person and manage initial client referrals for the residential program and other programs when assigned within 24 business hours.
- Conducts a thorough assessment based on referral and face-to-face meetings with potential residents in order to determine if the program will accept or defer the child.
- Develops and maintains an efficient referral process, ensuring all critical information about a child is received prior to consideration for admission.
- Maintains current knowledge of residential hall availability and fit through regular review of expected openings and maintains regular communication with hall staff and leaders.
- Facilitates all activities on the day a youth is admitted to the program and creates a welcoming atmosphere when they arrive and introduces the youth to all staff, explaining their role and who to go to for questions.
