Position Summary/ Department Summary:
Provides education and emotional support to patients and helps guide them through the healthcare system. Serves as a single point of contact for referring physicians, patients and caregivers to provide resources and assistance with accessing supportive care services within BCH and in the community. Key Responsibilities: Meets with/calls patients and families to review the implementation of care plans. Refers patients to services frequently used in a clinic and specialized services as needed. Explains hospital/community- based resources to patients and families, providing documentation.
Education: High School Diploma / GED is required. Bachelor's Degree is preferred. Experience: A minimum of one year of closely-related experience. Experience with the clinic population or work with community resources is preferred. The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting. Any base pay offer provided depends on skills, experience, education, certifications, and a variety of other job-related factors.
