Inova Fairfax Hospital is looking for a dedicated Registered Nurse (RN) Patient Care Navigator to join the Transplant Infectious Disease team. This role will be full-time, day shift: Monday - Friday, 8:00 a.m. - 4:30 p.m. (must be flexible). Will require rotation between inpatient and outpatient care at Inova Fairfax Hospital and the Inova Center for Personalized Health, located near Inova Fairfax Hospital.

Registered Nurse (RN) Patient Care Navigator - Transplant Infectious Disease Job Responsibilities:



  • Travel involved: rotation between inpatient and outpatient care at Inova Fairfax Hospital and the Inova Center for Personalized Health, located near Inova Fairfax Hospital.
  • Guides patient and family through the health system from diagnosis, testing, treatment and follow-up care to assist patients with navigating the continuum of care.
  • Coordinates a patient's plan of care with the Transplant Infectious Disease team which may include diagnostic test coordination and specialty medicine coordination and scheduling, patient education and knowledge assessment for complex treatment regimens.
  • Assists patients and families with accessing treatment resources by advising of options and engaging appropriate resources.
  • Manages complex cases effectively and efficiently. Collaborates with multidisciplinary teams to address patient questions and responses about the care process and treatment options.
  • Participates in obtaining quality outcomes such as decreasing patient infection and readmission rates.
  • Develops/maintains reliable systems to document, track and monitor patient data.
  • Performs other duties as assigned.


Minimum Qualifications:



  • Education: Bachelors Degree in Nursing
  • Experience: 3 years of nursing experience
  • Certification: Licensed in the Commonwealth of Virginia as a Registered Nurse; Basic Life Support from the American Heart Association


Preferred Qualifications:



  • Experience: Telemetry, Transplant, Infectious Diseases
