Located along scenic waterfront property in Brooklyn, NY, our Menorah Center for Rehabilitation and Nursing Care is seeking a Physical Therapist to evaluate, assess and treat patients within our rehabilitation center. * Evaluate residents to assess appropriate Physical Therapy treatment and rehabilitation potential * Establish Appropriate Treatment goals and Plan of treatment for each resident. * Plan and administers treatment modalities in accordance with a resident plan of care * Documents screenings, evaluations, progress notes and care plan tracking forms * Maintains statistical data on patients * Provides guidance and supervision to assigned Physical Therapy Assistants * Bachelor's degree in physical therapy * Current NYS Physical Therapy license * 2 years of geriatric experience from a rehabilitation & nursing home facility is preferred
