GENERAL SUMMARY:

Responsible for meeting patient priority needs by assessing, participating in interdisciplinary care planning, providing and /or delegating care and evaluating patient responses and progress toward clinical outcomes. Contributes to an efficient and effective work environment through activities including professional communication and documentation, precepting and/or resourcing of others.

Supervises Licensed Practical Nurses and/or other support staff in the delivery of patient care as described in Nebraska Statute 38.

PRINCIPAL JOB FUNCTIONS:

1. *Commits to the mission, vision, beliefs and consistently demonstrates our core values.

2. *Initiates assessment and reassessment based on the patient's presenting problems/needs in accordance with Bryan Medical Center's procedures.

3. *Synthesizes pertinent initial and ongoing patient data to identify nursing priorities for care and desired outcomes/goals.

4. *Integrates appropriate standards of care into the patient's plan of care based on the diagnosis and patient-specific needs.

5. *Executes therapeutic and/or supportive interventions.

6. *Provides education for the patient and family to meet identified patient needs according to their needs, abilities, preferences and readiness to learn.

7. *Evaluates the patient's response to and effectiveness of interventions.

8. *Evaluates patient's progress toward established outcomes.

9. *Incorporates cultural considerations into the plan/delivery of care.

10. *Includes family, significant other, or identified support person in the care of the patient as appropriate.

11. *Documents patient care and related information according to Bryan Medical Center procedures for documentation.

12. *Provides care in collaboration with other members of the interdisciplinary team.

13. *Facilitates patient's transition to the next level of care.

14. Assumes responsibility in a shared or delegated assignment and supports team member's practices through timely communication.

15. Serves as a role model to colleagues, students and other care providers.

16. *Actively participates in continued learning opportunities and continued self-development pertinent to the work unit and duties of the job.

17. Integrates evidence based research findings into practice in accordance with policy and procedure.

18. Engages in department specific and organizational performance improvement initiatives.

19. Appropriately utilizes all safety equipment when indicated and as defined by organizational procedure (i.e. lifting equipment, PPE, sharps, etc.).

20. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in field of expertise.

21. Participates in meetings, committees and department projects as assigned.

22. Performs other related projects and duties as assigned.

ADDITIONAL JOB FUNCTIONS:

In addition to the principal job functions contained on the primary job descriptions, the following duties are also required in the department noted above.



  1. Responsible for providing documentation and care support to a diverse population of patients across inpatient units.
  2. In collaboration with the bedside nurse, evaluates and documents patient response to nursing interventions and alters the plan as appropriate.
  3. Via video monitoring, provides support to direct patient care team through direct observation and documents as appropriate.
  4. Collaborates on assessments for assigned patients in partnership with bedside nurses as evidenced by complete documentation.
  5. Establishes an effective communication plan with bedside nurses and support staff to address changes in patient condition.
  6. Utilizes appropriate nursing diagnosis and data from assessment to derive realistic outcomes.
  7. In collaboration with bedside nurses, documents measurable goals on the plan of care and modifies the plan of care if indicated in order to maintain realistic patient-centered goals.
  8. Follows the organization's chain of command in appropriate circumstances.
  9. Collaborates with all members of the health care team including providers when planning patient care and documents accordingly.
  10. Demonstrates effective interpersonal skills in collaborating with the interdisciplinary team including nurses, physicians and ancillary staff.
  11. *Completes the admissions navigation process on scheduled, unscheduled or direct patient admissions on patient care units and the Emergency Department.
  12. *Supports the patient discharge process in terms of finalizing discharge orders, including medications; makes follow-up appointments, calls in prescriptions. May provide patient instructions and document discharge teaching.
  13. Assesses patient/family readiness for discharge planning and makes appropriate referrals.
  14. Observes for safe medication administration and performs dual signoff if appropriate.
  15. Implements and documents nursing interventions consistent with the plan of care.
  16. Effectively manages multiple responsibilities, prioritization, urgent responses and challenging situations.
  17. Provides documentation and support to safety/quality initiatives.
  18. Displays proficiency utilizing monitoring technology and Epic EMR.
  19. Reports to manager from primary unit of employment, in collaboration with Virtual Patient Care Services leadership.
  20. Assists with/performs other responsibilities as Virtual Nursing platform functionality expands and matures.



EDUCATION AND EXPERIENCE:

Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act. Must provide evidence of certification and/or instruction for practice area as required by State or Federal standards/regulations.



OTHER CREDENTIALS / CERTIFICATIONS:

Basic Life Support (CPR) certification required. Bryan Health recognizes American Heart Association (for healthcare professionals), American Red Cross (for healthcare professionals) and the Military Training Network.

PHYSICAL REQUIREMENTS:

(Physical Requirements are based on federal criteria and assigned by Human Resources upon review of the Principal Job Functions.)

(DOT) - Characterized as medium work requiring exertion of 35 to 50 pounds of force occasionally, and/or 10 to 35 pounds of force frequently, and/or a greater than negligible up to 10 pounds of force constantly to move objects.
