Description Field RN - Home Health Visits Position: Full-Time Registered Nurse (RN) Location: Indian River County About the Role Join Activa Home Health, an AccordCare Company, and make a meaningful difference in your community by delivering high-quality, compassionate nursing care to patients in the comfort of their own homes. As a Field RN, you'll play a vital role in ensuring patients' safety, well-being, and satisfaction while following individualized Plans of Care. Experience with WellSky/Kinnser and home health documentation is a plus! Key Responsibilities Provide skilled nursing care to clients based on individualized Plans of Care. Communicate promptly with the care team regarding changes in patient condition or emergencies. Deliver safe and effective nursing care within the RN scope of practice, which may include: Medication administration Wound care Infusion therapy Catheter care Patient transferring and positioning Collaborate with the Nurse Supervisor/Manager of Clinical Practice to ensure coordinated, comprehensive care. Demonstrate empathy, professionalism, and attention to detail in all interactions. Prioritize effectively, anticipate patient needs, and maintain a sense of urgency in care delivery. Qualifications & Experience Active RN license in the state of Florida Proven wound care skills Excellent communication, collaboration, and customer service skills Proficiency with electronic health record (EHR) systems, preferably WellSky/Kinnser 1 year of home health experience preferred Why Join Activa Home Health?
