Hourly Pay Range:

$28.47 - $42.71 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

  • Location: Swedish Hospital, Chicago
  • Part Time - 20 hours/week
  • Hours: Monday-Friday, 8:00am - 4:30pm
  • Required Travel: No

What you will need:

  • Bachelors of Science degree
  • Illinois state license as a Licensed Dietitian Nutritionist
  • Current registration as a Registered Dietitian by the Commission on Dietetic Registration
  • Current certification by the Certification Board for Diabetes Care and Education as a Certified Diabetes Care and Education Specialist (CDCES) -- preferred
  • One year experience providing diabetes education

What you will do:

The Clinical RD, Outpatient focuses on diabetes education and care in both the inpatient and outpatient environment. Inpatient responsibilities include providing bedside education for admitted patients with uncontrolled or newly diagnosed diabetes, offering tools and resources for blood glucose management, and referring patients for outpatient follow up. Outpatient responsibilities include conducting individual diabetes education sessions with patients.

The goal of this position is to bridge the gap between discharge to home and prevent avoidable diabetes-related readmissions. The RD is be expected to track the success and outcomes of this program.

This is a temporary grant-funded position - should financial funding for this role be discontinued at any time or cease beyond the duration of the grant period, the position may no longer remain active.

  • Responds to inpatient consults for diabetes education, provides bedside education that includes offering tools and resources for blood glucose management and discharge education for newly diagnosed patients with diabetes.
  • In the outpatient center; plans, directs, and provides nutritional diabetes care to patients and communicates recommendations to referring provider
  • Follows up with patients after discharge via phone call or in the Nutrition & Diabetes Center
  • Ancillary tasks such as attending special meetings and rounds as assigned, assisting with quality improvement audits and projects as assigned, conducting education classes when needed
  • Data tracking, data integrity and subsequent follow-up
