What you will do: The Clinical RD, Outpatient focuses on diabetes education and care in both the inpatient and outpatient environment. Inpatient responsibilities include providing bedside education for admitted patients with uncontrolled or newly diagnosed diabetes, offering tools and resources for blood glucose management, and referring patients for outpatient follow up. Outpatient responsibilities include conducting individual diabetes education sessions with patients. The goal of this position is to bridge the gap between discharge to home and prevent avoidable diabetes-related readmissions. The RD is be expected to track the success and outcomes of this program. This is a temporary grant-funded position - should financial funding for this role be discontinued at any time or cease beyond the duration of the grant period, the position may no longer remain active.
