Description MaineHealth Behavioral HealthBehavioral Health For a limited time MaineHealth is offering up to$5,000 Sign on bonus for all eligible Behavioral Health Case Managers! Current MaineHealth member employees are ineligible; former MaineHealth Members are ineligible until greater than 3 months separation from employment. Bonuses may be prorated based on schedule. ~ 40 Hours/week, Full-Time, Day shift ~ Maine MHRT/C certification is required We are looking to add a Case Manager to our Adult Services team. Our experienced case managers meet with patients and families to assess their needs. They also connect patients and families to resources and services that can support patient independence and quality of life. Our services promote physical and mental wellness as well as individual independence and success in their community. * Connecting patients and families with community resources * Helping patients and families navigate the social services and health care system * Coordinating services and communication between multiple providers * Developing and monitoring individualized treatment plans Summary The Behavioral Health Case Manager - Adult Services role is responsible for the provision of case management services and achievement of positive intervention outcomes for assigned clients.
