Responsibilities
  • Collaborate with the interprofessional team to delivery high quality person-centered care.
  • Assist patient with accessing community-based resources by supporting them with applications, transportation needs and follow up on any referrals made to ensure patient utilizes identified resources.
  • Enhance patient's ability to effectively communicate with healthcare provider
  • Works with staff and providers to facilitate a care plan and communicates with them regarding the status of efforts
  • Support patient understanding of educational materials and resources about diabetes
    • Support patient understanding of educational materials and resources about diabetes
    • Reinforces standards of care for diabetes including preventative care, annual monitoring and follow up care
    • Reviews medication plan with patient to confirm understanding and makes appropriate connections with prescribing provider when necessary
    • Assists patients with recognition of the barriers and implementation of strategies for healthy eating
    • Support patients understanding of and importance of monitoring tools and devices
    • Assists patients to overcome barriers to routine physical activity and connects them with appropriate resources

Qualifications
  • CMA, LPN, Pharmacy Technician, EMT/Paramedic
  • Diabetes Community Care Coordinator Certification required within 60 days of hire
  • Community Health Worker certification within 90 days of hire
  • Mandatory Reporter certification
