Overview
The Clinic Nurse utilizes a systematic approach to nursing by incorporating assessment, planning, implementation and evaluation in the practice of nursing. Provides healthcare to individuals, families, and communities within the RN scope of practice as defined by the Illinois Board of Nursing. Qualifications Certifications: Licensed Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR)Illinois Department of Financial and Professional Regulation (IDFPR); Basic Life Support (BLS) within 30 days - American Heart Association (AHA)American Heart Association (AHA), Education: Associate's Degree, Work Experience: Responsibilities Provides and facilitates competent health care of patients and families in area of practice. Ensures adherence to established standards of care and scope of practice. Ensures documentation of care is complete and accurate. Provides health and wellness education to individuals and groups. Acts as preceptor and supports the development of other staff and healthcare students. Provides assistance for initiatives designed to improve the quality of care or clinical outcomes. Assists director in the development of departmental standing orders. Coordinate with patient scheduling, referrals, intake and provide continuity of team resources. Provide care coordination for all ambulatory Palliative Care patients. Communicate with PCP and other team members to ensure continuity of care within the Carle system. Room patients, collect pre-visit symptom assessment for outpatient clinic. Complete preauthorizations and needed insurance documentation. Assist with tracking program quality metrics and data collection if needed Be the point of contact for Palliative Care team. Be available to answer phone calls and emails. Help coordinate patient flow, consults, and other processes. Delegate appropriately to office coordinator Make contact with patients who are new to the palliative care service to initiate enrollment into the program, assess current needs, and schedule needed appointments mostly through telephonic outreach Round on inpatients that are being followed by the palliative care service. Documents these interactions within EPIC. Conduct follow up calls for patients per provider instruction to ensure they have what they need, and assess overall condition. Documents accordingly using standardized templates that ensures proper data is gathered. Participate in interdisciplinary team meetings for inpatient and ambulatory settings as well as morning huddles with all providers. About Us Find it here. Compensation and Benefits The compensation range for this position is $30.39per hour - $52.27per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. The actual compensation offered a candidate will be dependent on a variety of factors including, but not limited to, the candidate's experience, qualifications, location, training, licenses, shifts worked and compensation model.
