The MGB mobile health service is an innovative program dedicated to increasing access to quality healthcare for historically marginalized communities in the Greater Boston area. The team in their mobile health van goes to different communities for pop up clinics to bring the care to where the community can best access it. The Nurse Practitioner is an integral member of the team bringing these health care services which include colorectal cancer screenings, vaccinations, cardiometabolic screening/treatment, and substance use disorder screening/treatment. This position allows the NP to meet the patient, where they are at, in their local environment to best directly assist them with their health care needs.
The NP is responsible for providing day to day clinical care to the community and supervision of the clinical members of the mobile team.
Together with the Mobile Operations Manager, the NP staffs several clinics each week across multiple locations and may be required to help set-up, staff management, and patient service. Because each clinic takes place in community locations with no immediate clinical back-up, it is vital that the NP is experienced and proficient at preventing or managing complex and/or emergency patient care problems.
The Mobile NP will work closely with the Senior Medical Director and the Mobile Operations Manager to develop and expand mobile health service offerings in the community. It will be important for the Mobile NP to have an interest in Community Health and be willing to explore new and innovative opportunities for providing service with the goal of overall improvement of health standards in our communities.
The Mobile NP reports directly to the Senior Medical Director for the mobile team and other nursing leaders for the organization
This is an in-person role. Some mobile clinics are scheduled during the evening and on weekends to provide greater access for community members who work during the day

Clinical Job Responsibilitiesinclude but are not limited to the following activities:

1.Functions independently and uses EPIC to provide care for all patients seen by the mobile team.

2.Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or emergency patient care problems.

3.Maintains and practices in accord with practitioner guidelines per state and hospital regulations.

4.Works in other mobile clinical roles if needed due to a staffing shortage or high patient volume.

5.Provides primary care services in affiliated settings as needed.

*Administers medications and injections in compliance with state law and clinic guidelines.

*Actively supports patient safety program; adheres to applicable patient safety standards.

*Instructs patient/family regarding medications and treatments. Educates patients regarding health and illness prevention.Recommends community resources as needed to meet patient and family needs.

*Maintains and reviews patient electronic medical record and other pertinent information. Prepares written documentation as required in the patients' medical record following standard clinical practice.

*Collaborates with physicians in managing acute and long-term medical needs of patients.

*Provides clinical results and consultation to patients over the phone in follow-up to care provided during the mobile clinic.

*Treats all patients courteously and with dignity and respect at all times.

*Maintains patient confidentiality

Qualifications

*Bilingual in Spanish preferred

*Current Massachusetts Licensure as an NP required

*Master's Degree in Nursing (MSN)

*One year of clinical experience required

*Ability to work morning, afternoon, and or evening hours in a given week

*Ability to commute to different parts of Boston for clinic sessions

*Ability to work occasional weekends as needed
