Provides clinical pharmacy services including but not limited to assessment, monitoring and optimization of patient specific pharmaceutical care. Serves as a departmental and organizational lead in area(s) of expertise and provides educational programming and staff development to physicians, pharmacists, nurses and other practitioners in assigned specialty area. Oversees organizational development and revisions of medication therapy regimens, protocols and processes. Serves all patient populations including neonates, pediatrics, adolescents, adults and geriatrics.

PRIMARY DUTIES AND RESPONSIBILITIES

  1. Performs patient specific clinical pharmacy consults in assigned area. Responsibility includes evaluation of all medication-related needs and development/implementation of pharmaceutical care plans that optimize medication use.
  2. Monitors specific blood and laboratory tests including electrolytes, renal function, hepatic function, culture and sensitivities, and fluid balance to determine effectiveness or clinical pharmacy interventions.
  3. Provides patient education and medication counseling to ensure appropriate and safe use of medications.
  4. Provides and oversees the provision of drug information and in-services to physicians, nurses, and pharmacists.
  5. Provides training and in-services related to the implementation and maintenance of various clinical pharmacy processes.
  6. Develops order sets, Nurse Information Sheets, Patient Information, and various other paperwork as needed.
  7. Works closely with Staff Development Supervisor to develop, plan, organize, implement and assesses effectiveness of various educational programs.
  8. Develops Drug Usage Evaluation activities for drugs within areas of clinical expertise.
  9. Coordinates collection of quality improvement initiatives and reports data at various multidisciplinary meetings.
  10. Participates in and supports departmental patient and medication safety initiatives.
  11. Performs new drug evaluation and formulary/class reviews and advises department on positioning of new and existing formulary agents.
  12. Precepts pharmacy students and residents experiential clerkships and rotations.
  13. Assists in adverse drug event review and analysis when requested. Provides recommendations for improvements to processes and policies to prevent future incidents.
  14. Performs the duties of Staff Pharmacists when necessary.
  15. Practices proper safety techniques in accordance with hospital and departmental policies and procedures. Immediately reports any mechanical or electrical malfunctions, unsafe conditions, or employee/patient/visitor injury-accident to Manager.
  16. Demonstrates service excellence behaviors routinely in encounters with all customers, i.e., coworkers, patients, visitors, physicians, etc.
  17. Other duties as assigned by supervisor.

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION REQUIRED

  1. Graduate of an accredited School of Pharmacy.
  2. Doctor of Pharmacy degree OR six (6) years of equivalent knowledge base/experience.
  3. Completion of a post-Doctor of Pharmacy Degree clinical residency of one (1) year OR equivalent clinical experience.
  4. Three (3) years previous hospital pharmacy practice experience.
  5. Ability to develop, organize and implement various programs.
  6. Excellent written and oral communication skills.
  7. Evidence of an ability to work well with others, a team player.
  8. Evidence of teaching skills.

KNOWLEDGE SKILLS AND ABILITIES/LICENSE OR CERTIFICATION PREFERRED

  1. Previous experience in staff development
  2. Clinical residency

LICENSE OR CERTIFICATION REQUIRED BY THE STATE AT NORTHSIDE HOSPITAL

  1. Current licensure as a Registered Pharmacist in the State of Georgia
