Program Manager, Health Equity

Position Overview:
The Program Manager's primary responsibility is to lead the programmatic activities of the Connect2Care (C2C) program. Connect2Care's health equity strategy is focused on connecting chronically ill, at-risk low-income New Yorkers to health and social services, and improving access to health resources, screenings, out-of-hospital preventive care and social support services for New Yorkers. The program will focus on addressing key health issues, including key diseases such as diabetes, cardiovascular disease, maternal morbidity, obesity and hypertension, and provide direct preventative health screening services to low-income and under-insured individuals.

The Program Manager will focus on increasing the capacity of community-based organization (CBO) partners to deliver culturally competent health care interventions, including the training of Health Advocates, to strengthen the capacity of each CBO Partner to provide health advocacy and preventative care services on-site at their facilities.

The Program Manager will collaborate with CBO Partners, clinical providers, and other internal and external stakeholders on the management, administration, and implementation of the programmatic inputs needed to support the C2C Program.

For Connect2Care, the Program Manager is responsible for working directly with community-based partners on meeting grant deliverables, including timely submissions of quarterly progress reports, budgets and expense reports through our internal fiscal management system, and tracking program outputs in our data management system.

Primary Responsibilities:
  • Serve as a point of contact for Connect2Care partners to provide technical assistance and guidance to ensure partners can meet deliverables and remain in compliance.
  • Implement strategies for increasing access to preventative health resources among vulnerable populations.
  • Foster strategic partnerships with community-based organizations to promote health equity.
  • Assess and analyze best practices by researching the current and emerging trends in health outreach and programming.
  • Develop outreach materials for C2C partners.
  • Collaborate with the Senior Director, through UWNYC's Invoice Management System (IMS) to provide all invoice documents and manage reporting of expenditures to NYSDOH.
  • Prepare progress report documents for C2C partners.
  • Identify training needs, develop training materials, and provide ongoing, necessary training to CBO partners to assist them in fulfilling the objectives of UWNYC's C2C program.
  • Stay informed of health policies on the local and national level.
  • Conduct training on Connect UWNYC platform to ensure proper submission of SDOH screening and referral data.
  • Conduct site visits with CBO partner organizations to monitor performance.
  • Review interim and final grant reports for C2C partners.
  • Work independently and collaboratively with internal and external partners.
  • Participate in all meetings, webinars etc. related to C2C programs as requested.
  • Plan, organize, and execute other grants administration tasks and administrative tasks as assigned.
  • Use Results Based Accountability framework to track team goals.
Team Management

  • Communicate the benefits of change to encourage team buy-in and minimize resistance.?
  • Work with Health Equity leadership team and staff to recognize internal and external opportunities and solutions and define and execute appropriate strategies to support them.
Skills, Knowledge, and Experience:
  • Master's degree in social work, public administration, public health, sociology or related fields with a minimum of 2 years work experience or a bachelor's degree in sociology, politics, public administration or related social sciences, with a minimum 4 years of related work experience.
  • Knowledge of, and experience with, nonprofit grant management and program administration, and/or demonstrated knowledge of preventative healthcare and other health and social resources.
  • Strong team orientation, relationship-building and negotiation skills, and ability to collaborate with diverse groups of people.
  • Strong presentation, public speaking and meeting facilitation skills.
  • Excellent organizational, project management, time-management, and negotiation skills with ability to collaborate with diverse groups of people.
  • Detail-oriented, self-motivated, driven, proactive, collaborative, flexible, and good-natured.
  • Strong technology aptitude skills including knowledge of enterprise software systems, and able to easily learn new programs and technologies.
  • Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Microsoft Forms, Outlook).
  • Experience in grantmaking or providing technical assistance for CBOs a plus.

  • the individual's skill sets, experience and training;
  • licensure and certification requirements;

With that said, as required by local law, Vaco by Highspring believes that the following salary range referenced above reasonably estimates the base compensation for an individual hired into this position in geographies that require salary range disclosure. The individual may also be eligible for discretionary bonuses.
