Description Mid Coast HospitalProfessional - Clinical ~ 8 Hours/week, Limited Part-time, Day shift ~ Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW) is required The Palliative Care Social Worker is an integral member of the Palliative Care Interdisciplinary Team which delivers patient- and family-centered care to individuals of all ages living with serious illness. The goal of palliative care is to maximize the quality of life and align the individual's medical care with what matters most to them. Key responsibilities of the Palliative Care SW include evaluating the psychosocial needs of patients and families, providing targeted counseling including anticipatory grief support, facilitating family meetings and goal-setting discussions, making referrals to community resources, and engaging in effective interdisciplinary team collaboration. This role will work primarily in the outpatient clinic in Brunswick and may also meet patient and family needs at the hospital or in other specialty practice settings through palliative care outreach (in person and/or via telehealth).
