Summary:

The Minimum Data Set (MDS) Coordinator is responsible for conducting and coordinating the completion of the Resident Comprehensive Assessment (RAC), which includes the Minimum Data Set (MDS - part of a federally mandated process for clinical assessment of all residents in Medicare or Medicaid-certified nursing homes), Resident Assessment Protocols (RAPs), and the Resident Assessment Protocols (RAP's) summary sheet as well as the coordination of periodic comprehensive, interdisciplinary assessments upon admission, annually, and when there is a significant change in the resident's condition.

Detailed responsibilities:

  • Assess and monitor patient care and well-being upon admission, annually, and when there is a significant change in the resident's condition to ensure the facility provides consistent quality care.
  • Develop resident goals for improving treatment and care plans.
  • Evaluate the resident care that all patients receive.
  • Follow up with patients on any complaints or incidents reported.
  • Meet with patient caretakers and families to discuss the patient's condition and treatment plans.
  • Approve resident applications for long-term care facilities.
  • Continually provides oversight and educates nurses and interdisciplinary team members regarding MDS protocols and accurate care planning interventions per State and/or Federal regulations.
  • Responsible for timely and accurate submissions of MDS.
  • Monitor patients' medical insurance and payment plans; track Medicare benefit days, validate daily Medicare census, and coordinate the information with the business office.
  • Continually review and update resident care plan, implementing new strategies to enhance residents' quality of Life as required.

Credentials with Equivalent Qualifications:
Essential:
* Registered Nurse - California or Licensed Vocational Nurse - California

Non-Essential:
* RAC Certification by AAPACN

Competencies and skills:

Essential:

* Must have good communication and customer service skills; ability to work well with others and take direction; ability to work under pressure to ensure timely review, correction and reporting of the payroll. This position will also assist account payable and accounts receivable with imputing data, preparing bank deposits, etc.

Educational with Equivalent Qualifications:
Essential:
* Graduate from accredited school of nursing

Experience with Equivalent Qualifications:
Essential:

* 1 Year of Patient Driven Payment Model (PDPM) reimbursement

* 2 Years with knowledge of State and Federal regulations

Non-Essential:

* ICD 10 Coding
