Responsibilities

Fort Lauderdale Behavioral Health Center (a UHS facility), is a 182-bed behavioral health facility located in Oakland Park, FL (Fort Lauderdale, FL), has been providing quality health care to the residents of South Florida. FLBHC provides inpatient and day treatment for children, adolescents, adults, and senior adults. Our professional staff individualizes care to uniquely fit the intensity and severity of each client's case and their family's needs. Because crisis occurs at all times of the day and night, our services are provided 24 hours a day, 7 days a week.

Patient Care (Step-Down) Navigator

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. He/She will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.

Qualifications

Education: Bachelor's Degree in mental health/behavioral sciences field. Master's Degee, Registered IN preferred.

Experience: Minimum two (2) years' experience working in psychiatric/mental health field.

Licensure: CPR/BLS certification

Crisis Prevention Intervention (CPI) required
