The Resident Service Coordinator operates as a member of the leadership team and as such, is

responsible to set the example for the staff of the mission and core values of the Felecian Sisters to ensure that the

highest degree of quality resident care is delivered to our residents. The Resident Service Coordinator is

responsible for overseeing the development, implementation, supervision, and ongoing evaluation of the

Social Services Department designed to meet and assist residents in attaining or maintaining their highest

practicable well-being. This includes identifying the need for medically related social services and

ensuring that these services are provided in accordance with State and Federal regulations.

The Resident Service Coordinator will complete and/or direct/delegate the completion of the Resident

Service component of the comprehensive assessment. These goals and interventions will be

individualized to match the skills, abilities, and interests/preferences of each resident in compliance with

Federal and State regulations, to include identifying and promoting individualized, non-pharmacological

approaches to care that meet the mental and psychosocial needs of each resident.

Experience and/or Pre-requisites

Experience in PCC r equired

Experience with ISPS r equired

Experience with Family Care-Community Care-Medicaid r equired

BIMS experience in PCC p referred

Must be an effective leader with proven experience.

Minimum 1 years' of health care setting working directly with individuals Knowledge and ability to use designated software

Must have the ability to read, write, speak, and understand the English language.

Minimum 1 years' of PCC

Must have sound knowledge of Federal and State regulations related to resident's rights and

quality of life requirements.

Must plan and organize work assignments in an ever-changing environment.

Must make independent decisions when circumstances warrant such action.
