DESCRIPTION OF WORK

An Aging Care Manager 2 receives both formal and informal training and becomes knowledgeable of the rules and regulations of programs provided by the Agency. After training, you will be able to perform intake and referral to services duties, in addition to completing initial intake and document information from consumers. Your communication skills will be key as you will have the opportunity to be the initial contact person for consumers, family members, or professionals requesting information regarding services. This includes explaining and describing availability, eligibility, and program limitations or restrictions. You will maintain a caseload of consumers providing ongoing care management and coordination of services with other community resources as needed. Our ideal candidate must have the ability to manage multiple tasks and demonstrate close attention to detail, which includes completion of paperwork, work assignments, and meeting deadlines. As abilities and understanding of available programs progress, this professional will be assigned increased responsibility for arranging consumer services. Work Schedule and Additional Information:
- Full-time employment, 37.5 hours per week, with on-call hours when required.
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
- Some out of county overnight travel for meetings or training may be required.
- Telework: You will not have the option to telework in this position.
- Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Successful completion of the County Social Casework Intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
