Attractive Compensation Package

Role of the RN Case Manager
As a Hospice Case Manager for Amedisys, you will have a special opportunity to provide, coordinate and direct the provision of hospice nursing and palliative care to terminally ill patients in their home and wherever they call home. Our Hospice nurses will closely collabo rate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high-quality end-of-life care for the patient. We service an adult population providing support to patients and families.
Nuts & Bolts of the Job
Hospice RN Case Managers work Monday - Friday with a potential on call and Holiday coverage schedule. Case Manage 15-18 patients Attend Interdisciplinary Team Meetings Contribute to team huddle calls and receive updates on your patients daily
Provide Case Management:
* Assign Aide Visits
* Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team
* Develop and nurture relationships with facility staff and leaders throughout the community

Orientation, Onboarding & Training
You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however, is individualized according to your needs and experience.

Responsibilities

  • Performs patient assessments and collaborates with the care team to develop and implement a plan of care.
  • Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
  • Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Teaches patients, families, and caregivers appropriate patient care techniques.
  • Regularly communicates with clinical manager and care team.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Other duties as assigned.

Qualifications

  • One year of nursing experience.
  • Current RN license, specific to the state(s) you are assigned to work.
  • Current CPR certification.
  • Valid driver's license, reliable transportation and liability insurance.
